JOB SUMMARY
The incumbent, reporting to the Provincial Medical Manager and working closely alongside the nurse mentor(s), will be expected to provide expert medical care, advice, diagnosis and treatment of all patients in care at all AHF-supported clinics within the allocated district/subdistrict in the province, and sometimes remotely to other provinces where necessary. The incumbent must monitor the medical programmes in AHF- supported sites and support the roving team in the identification of opportunities for improvement and in the development of quality improvement plans in all global health strategies in HAST. They will also be required to report to senior management regarding quality clinical initiatives and programmes within the province. The MO will lead clinical audits (incl. mortality audits) in the supported facilities.
Position is to be based at Mpumalanga AHF Office - Witbank
ESSENTIAL DUTIES & RESPONSIBILITIES/KEY PERFORMANCE AREAS
Medical Responsibilities include:
- Provide medical services to clients in AHF and supported clinics
- Supports HIV, TB and STI case finding, including index case identification, TB case identification, STI screening, etc.
- Ensure effective patient monitoring and retention, including CD4 coverage, TPT uptake, AHD management, VL coverage and suppression, tracking tools, etc.
- Actively monitor and manage viral load suppression, TB outcomes and provide advanced level care to complicated cases within the allocated facility(-ies).
- Detection and management and appropriate referrals (where applicable) for cases with co-morbidities, including NCDs, Cervical CA and opportunistic infections.
- Management of maternal (PMCTC, EMTCT), paediatric and adolescent clients.
- Provides clinical direction and support and general management oversite
- Ensures that all patients access various diagnostic tests. Analyses and evaluates medical procedures and diagnostic tests that promote good clinical outcomes in accordance to standard guidelines.
- Responsible for maintaining delivery of high quality medical care and efficiency of all services provided within the program.
- Support the Provincial Medical Manager in ensuring that the facilities align with DoH policy and reporting tools
- Support the Provincial Medical Manager and preventions team in ensuring that all patients are retained in care.
Key Performance Areas:
- The provision of promotive, preventive, diagnostic and therapeutic services as per Primary Health Care package of services, Adult Primary Care (APC) and Advanced Clinical Care (ACC), taking into consideration the burden of disease of the community being served by the Facility.
- Use of clinical algorithms as per latest updated STGs/EDL, APC and ACC Clinical Guidelines and advanced level care for the management of chronic communicable diseases such as HIV, AIDS and TB. Management of STI’s, HIV/AIDS associated opportunistic infections, minor ailments and other communicable diseases.
- Use of clinical algorithms as per the latest STGs/EDL, APC Clinical Guidelines and advanced level care for the management of identified non-communicable diseases within the scope of AHF-supported sites.
- Provision of rehabilitative services, such as adherence to referral protocols to all relevant points of
service delivery, including community-based rehabilitation services, CHC, district hospital and levels above.
- Assessments and recommendations of disability and other relevant social support grant applications.
- Deliver services as per the service package of the relevant health facility.
- Ensure compliance with the Essential Medicine List (EML) and relevant AHF formulary.
- Respond to emergencies and ensure adequate management and referral of such patients
- Participates in, complies with and leads Clinical Governance activities and initiatives, Quality Improvement Projects (QIPs) and programmes (incl. ORI and Food for Health), Medical Directors meetings to discuss relevant matters, including difficult cases and management issues, presentation of clinical cases, etc.
- Drives and conducts clinical audits and M&M reviews, including mortality and morbidity audits, and compiles related reports and recommendations; presents findings in monthly Provincial Clinical Governance meetings.
- Ensures and monitors that all clinic staff participate in scheduled staff and recommended CPD (continuous professional development) and CME (continuous medical education) activities in a manner that upholds peer interaction; presents topics and cases in CME meetings.
- Participates in clinical assessments and other related activities.
- Be active in the development of best practice, clinical guidelines and protocols, SOPs, operational research incl. protocol development, scientific abstracts, etc.
- Implements AHF clinical strategies and SOPs
- Support other (non-medical) AHF strategies, programmes and initiatives, such as Optimum Retention Initiative, Back in Care, Food for Health, advocacy and marketing activities, etc.
- Mentors and gives in-service training to clinical and non-clinical teams within the facility, AHF supported sites and districts, including delivering webinars and similar educational activities.
- At the request of and cost to the Department and AHF, attend training, orientation/ induction and meetings.
- Serve relief for the Provincial Medical Manager in his/her absence, whenever required.
General duties are:
- General day to day management of patients and support to the clinical team and all roving team members
- Participate in regular team meetings.
- Any other duties assigned by supervisor.
Supervisory Responsibilities:
Provides guidance, supervisory support and general management to clinical teams.
Ensures that the clinical staff have staff have all the information and clinical support that they need.
Carries out supervisory responsibilities in accordance with AHF’s policies and applicable guidelines/prescripts. Some of the responsibilities include interviewing, hiring, and training employees; planning, assigning and directing work, appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems.
Education and/or Experience
Qualified Medical Doctor with 3-5 years’ experience in HIV related field and at least 3 years of clinical management exposure.
